New Patient Paperwork
To simplify your first visit to Panacea, we request that all patients fill out the required new patient paperwork prior to their appointment. To access the forms, please click the below button. Please do not hesitate to reach out to us if you have any questions while completing the various forms.
Consultation Fee & Cancellation Policy
Our in-person consultation fee is $50.00. Virtual consults are performed with our compliments. Fees are required to be paid at the time of scheduling. We schedule our appointments so that each patient receives the right amount of time so we can ensure all of your questions are answered.
If you elect to move forward in scheduling your surgery on the date of your consultation, your $50.00 consultation fee will be applied towards your surgery deposit. As a courtesy to our office as well as to those patients, or clients, who are waiting to schedule with the physician, please give us a sufficient notice of at least 72 hours’ notice.
If you do not cancel or reschedule your appointment with at least a 72 hours’ notice, your $50.00 consultation fee will not be transferable toward a future appointment.
You will need to pay another consultation fee to reschedule.

Post-Surgical Lymphatic Drainage Treatment
Treatment costs and fees for lymphatic drainage massage. Dr. Singh and Dr. Abboushi recommend treatment 2-3 times a week for 6 weeks post-surgery.
